Where can I get the Affidavit form that needs to be filed with county clerk after the divorce judgment?

My divorce judgment was signed by the judge. Now, it says that I have to give a copy of judgment to my wife and then file an affidavit with the court that I have done that. Where can I get this Affidavit? I filed for Divorce in New York City. It's not available on NYS Supreme Court's website along with other divorce forms.

*** An affidavit is simply a statement of fact. Copy the information found at the top of your divorce papers, including the case number. Type out a statement saying when and where you provided the papers to your wife. Sign it in front of a notary, than register it with the court. Easy Peazy.
